Getting Fluoride To Estimate For Every Age In Eaglesoft

Fluoride Varnish for children up to age 14 is typically covered at 100% under most Preventative categories of coverage.  So, what about those 15 years old and older who typically have no coverage? 

So here is how I would set this up:

1)    I set up three new Service Types under List>General Setup>Service Types

o  Preventive – Fluoride 0-14

o   Preventative – Fluoride 15-18

o   Preventative – Fluoride 18 & Up

o   I set the 0-13 with a default percentage of 100% and both the 15-18 and the 19 & Up with a default percentage of 0% and no deductible to apply for all.

2)    I create two new Admin Service Codes   

a.    Service Code: T1206 (the “T” stands for teen)

b.    ADA Code: D1206

c.     Description: FLUORIDE - TOPICAL VARNISH 15-18

d.    Master Standard Fee: Set to the same as your Standard Fee

e.    Submit to Insurance: Yes

And

a.      Service Code: A1206 (the “A” stands for adult)

b.      ADA Code: D1206

c.      Description: FLUORIDE - TOPICAL VARNISH 19 & Up

d.       Master Standard Fee: Set to the same as your Standard Fee

e.      Submit to Insurance: Yes

Then

f.        Edit your existing (old) D1206 Service Code and re-name it FLUORIDE - TOPICAL VARNISH 0-13
